URINARY TRACT INFECTIONS
DIAGNOSIS

Unexplained fever > 38°C
-
Infants with non-specific illness, failure to thrive, feeding difficulties
-
Unexplained vomiting or abdominal pain
-
The frequency of micturition, dysuria or enuresis.

Prolonged jaundice in the newborn
-
Suspected sexual abuse
-
Haematuria / Hypertension
If Leucocytes & Nitrates negative but blood or protein are positive then, UTI is unlikely but always send for culture if clinical suspicion
CRITERIA FOR DIAGNOSIS
-
A pure growth of > 100,000 organisms.
-
Any growth in Suprapubic aspirate or in a catheter specimen.
Remember-
- The presence of a ‘mixed growth’ does not exclude UTI.
TYPES OF UTI
Acute pyelonephritis or upper UTI:
It is defined as

Fever> 38° C and positive urine culture
-
Fever < 38° C with loin pain and tenderness plus positive urine culture
Cystitis or lower UTI:
-
The absence of systemic signs and symptoms but positive urine culture
