DIAGNOSIS

  • Unexplained fever > 38°C

  • Infants with non-specific illness, failure to thrive, feeding difficulties

  • Unexplained vomiting or abdominal pain

  • The frequency of micturition, dysuria or enuresis.

  • Prolonged jaundice in the newborn

  • Suspected sexual abuse

  • Haematuria / Hypertension

 

If Leucocytes & Nitrates negative but blood or protein are positive then, UTI is unlikely but always send for culture if clinical suspicion

 

CRITERIA FOR DIAGNOSIS

  • A pure growth of > 100,000 organisms.

  • Any growth in Suprapubic aspirate or in a catheter specimen.

 

Remember-

  • The presence of a ‘mixed growth’ does not exclude UTI.

 

TYPES OF UTI

 

Acute pyelonephritis or upper UTI:

It is defined as

  • Fever> 38° C and positive urine culture

  • Fever < 38° C with loin pain and tenderness plus positive urine culture

 

Cystitis or lower UTI:

  • The absence of systemic signs and symptoms but positive urine culture
