HEAD INJURY- HISTORY TO CONSIDER

In any case of head injury always consider the following history.

If suspicion of neck injury always use the cervical collar and then begin the questioning.

Use all available sources to get the history from parents, bystanders, family
Ask about the following-
Mechanism of injury
Time of injury
Loss of consciousness

Unconsciousness may mean the injury was at least moderate severity

Also, record the time of amnesia if present
Headache
- The position of the headache and severity
Nausea and vomiting
Limb weakness
Paresthesia
Diplopia
CSF leak from nose or ears
Bleed from eyes/ ears/ nose
Neck injuries
Any other injuries
Past medical history
-
Document pre-existing illness like epilepsy, cardiac conditions, bleeding problems.
Drug intake
Medications that the child takes
-
Any alcohol or drug abuse- this may hamper the consciousness level
Tetanus status
